The major theme in this episode is Aunt Ruth and her weird new neighbor. As for Ruth, she's showing many signs of dementia and she's naturally very worried. Is it Alzheimer's or common dementia or perhaps something else? As for the neighbor, she's a real weirdo who has a secret that involves some stolen mackerel. Why would she do this? See the show. The other main theme is the party Al Large is planning for his fiance...and it coincides with the Doc's birthday.
This is a pretty good episode with quite a few surprises. So, it being less predictable than usual is a very good thing. Well worth seeing.
