- You come across the house of a child murderer, child molestation is implied. Never shown or gone into much more detail than that. The implications are still very creepy and disturbing.
- Nothing graphically sexual but some enemies come across as sexual in a very disturbing way.
Violence & Gore

- Lots of disturbing and graphic imagery including suicide, self mutilation, murder and gore. Several monsters you fight in the game look deformed and have disjointed limbs and walk and sound like they're in agony.
- There's a part where you enter an appartment building and find black bags with blood around them. It is mentioned and heavily implied dead children are in them.
- Lots of relentless horrific violence and gore.
- Simon is seen with cuts all over his wrists as he injects morphine needles which give him health.
- Some environments are covered in blood.
- There is a heavy theme and implication of suicide and depression found in this game
- The gore and blood can look very realistic sometimes.

- You take morphine syringes throughout the game to regain health however it makes the character you play as addicted to it and in need of morphine and painkillers.
- Simon smokes cigarette in cutscenes.
- Beer bottles and ashtrays can be found in empty apartments.
- Simon becomes addicted to opiates.
Frightening & Intense Scenes

- This game is full of terrifying monsters and imagery.
- Lots of jump scares and terrifying sounds.
- This game can get extremely graphic.
- Despite the horrific imagery it also deals with dark subjects such as mental health, trauma, drug addiction, PTSD, child murder, loneliness and depression.
- The tone and atmosphere of this game is very dark and depressing.
- If this where a movie it would be rated NC-17 for strong bloody violence, dark and disturbing images throughout and some drug abuse.

- Simon stomps the doctors head into the ground, this is very brutal.
Frightening & Intense Scenes
- Simon has romantic feelings for Sophie, he is rejected by her and she then kills herself by throwing herself off a building.
