- Small fry Juan Rivera is caught by a police helicopter in the Paiute India reserve, driving a truck with surgical waste, harvested organs and a knave's body. Juan seems the previously unknown accomplice of the elusive organ trader Dr. Jekyll, but claims to be anonymously hired by 'El doctor' for a single dump. The boy is motel night watchman Gomez's son Eduardo, and died from a surgery where bleeding couldn't be controlled. Illegal Mexican drugs point to suspected illegal 'doctor' Jeffrey Hughes. Prostitute Karen Jones (23) in a casino hotel corridor, underwear only. Greg finds her rival colleague Jillian Rose's corps is found hidden in Karen's hotel bed. - Chris Cavaliere, Nick and Langston investigate one corpse, human remains and surgical material dumped in a remote area by a truck driver. The man is Juan Rodriguez and they believe he works with Dr. Jekyll. Meanwhile, Brass, Catherine and Greg investigate the case of a young woman found murdered in the corridor of the Mediterranean. Catherine finds her purse with a gun and they identify her as Karen Jones, from Phoenix. Brass interviews a bartender in the casino and he tells him that she was with another young woman at the bar. Greg watches a surveillance tape from the bar and notes that the unknown woman was flirting with a man at ATM. Two cases in Sin City tonight.
In the first Catherine, Greg, and Brass investigate the murder of a woman found dead in a hotel: Karen Jones. At first they suspect that she might be a hooker. They observe her on hotel video chatting with another woman and then working a guy at an ATM but she doesn't snag him. They later see her coming to the hotel in the early morning and getting off on the same floor on which she was found. They go "camping"covering her body in plastic and dusting for prints. They find one on her leg where she was dragged but don't get a hit.
Brass and Greg go to check out the motel where she was staying and discover the other woman she was hanging out with dead, stuffed into the box spring of her bed killed in the same manner at near the same time as Karen. They wonder if it was two killers but think it was proabaly just one, who killed this girl first.
They look for the second girl on the surveillance video and discover that she landed the ATM guy. Six minutes later she was taking off, not enough time for sex. Catherine deduces they weren't hookers and it wasn't about sex. She goes and pulls out an evidence bag and gets one of the girl's phones and looks at the texts. She deduces they were running a con in which one girl would steal a mark's pin number while the other swiped the credit card.
The guy from the ATM, an actuary named Fiore in town for a conference, already checked out. Catherine and Greg confront a cleaned room and investigate the only parts that stay the same: drawers, closets, vents. Inside the vent are Karen's silver stilettoes, wet and destroyed. They realize he tried to flush them. They examine the toilet and find a sequin. He flushed the dress. They discover it a few floors down stuck in a pipe.
They bring Fiore in and question him. He essentially admits to killing Karen in self defense when he confronted her about the con and she pulled a gun. He didn't report it because there would be a report with a 100 percent chance his wife, an heiress, would find out. Statistically speaking he figured he'd be better off killing her and pleading self defense.
Greg discovers grey fibers in the dress and Hodges says they come from a polyester box spring like the one in where the other girl was stuffed. They check out the trash can liner and discover a lipstick smudge, from where she was asphyxiated. They find palm prints. They go back to Fiore who confesses he followed her to the room to get his wallet and killed her when she wouldn't give up. Karen returned and found her dead and decided to back and try and extort money. But he still killed her in self defense he says. He talks a lot again about how statistically he was better off killing them. Catherine says he's going to get 20 years to life. He's confused since he says he works hard and contributes to the world unlike those women who were worthless criminals. Catherine corrects him and says they were human beings and that he's a worthless criminal.
In the other storyline a young boy named Eduardo is found dead in the desert. Along with his remains in trash bags there are pharmaceuitcals and discarded body parts of other people. Ray and Nick think the Jekyll/Hyde serial killer has struck again.
They check out the "vitamin store" where some of the pharmaceutical stuff came from by way of Mexico. The man who runs the place Hughes maintains that he's an herbalist but after Nick and Langston investigate the back room and discover human blood in the newly regrouted floor he confesses he went to medical school in Mexico. He says he's only trying to help his poor patients. He claims he knows nothing of the earlier Dr. Jekyll murders and they believe him.
(In the course of the investigation when Wendy confirms the blood on the floor she asks Langston for his DNA to toss out his information- mingled when he ground up the grout with his fingers. He, politely, refuses to give it to her. We also learn he works at a free clinic).
