Martin has to put up with PC Joe Penhale's brother Sam in more ways than one. Pauline has hired Sam to paint the reception room, which Martin doesn't agree with, but he also notices that Sam sometimes loses his balance or his motor coordination. He's primarily concerned about the possibility of a hereditary disease and when hypochondriac Joe hears about it, he panics. Louisa meanwhile hopes to move into old Mr. Routledge's house. The old man, who drives Martin mad with his constant complaining, desperately wants to move into a care home, but Martin won't agree to refer him. He inadvertently facilitates the move. Martin and Louisa are barely talking and his friend, Edith Montgomery, thinks he should be returning to London. Meanwhile, Pauline thinks boyfriend Al is too passive and predictable.
