- One male and one female enemy wanders around completely nude with their genitals exposed. Another innocent victim also appears completely nude.
- One of the main villains tases his own genitals as well as player characters'/mannequins' genitals.
Violence & Gore

- Throughout the game you and your team will have to go through pure nightmare fuel while trying to avoid mentally disturbed homicidal people as well as trying to avoid setting off traps that could have violent and sometimes fatal consequences.
- One of the ways you and your team can fight back is by throwing bottles and bricks at enemies. When it hits them a blood splatter effect plays. Not that graphic
- Multiple levels involve keys to progress. A TV screen gives away a certain shape. You must look for mutilated dead corpses with the specific shape tattooed into them. Once found you must hold the left mouse button to forcefully stick your hand in and search for the key that is located within them. Lots of blood.
- If you loose all of your health you will begin to bleed out. While bleeding out an ally can come to your aid by stopping the bleeding and getting you back on your feet. Or if an enemy catches you while in this state they can execute you in rather gruesome ways. Once dead you are out the game however an ally can revive you by using a syringe to get your heart started again.

- While the swearing is hard to hear there are indeed uses of F*ck, Sh*t, B*tch, Wh*re and more
Alcohol, Drugs & Smoking

- Whenever entering a game the facility will gas your character or drug them to make them see the enemies they will have to deal with in the upcoming trial.
- Throughout the trials there is an enemy that can gas you or you can set off traps that release said gas. The gas messes with your characters mental health ad once depleted you will develop psychosis which will cause you to hallucinate and see enemies that can attack you. After using an antidote your mental health will return to normal.
- One of the items you can use is adrenaline which is used by sticking a needle that comes with the item through your arm and injecting the substance into you. Doing this gives you a speed boost.
- You can use game currency to purchase prescriptions in the main lobby. Doing so will permanently increase or give you new abilities.
- The first game in the franchise that have explicit drug content.
Frightening & Intense Scenes

- The game has a very intense feeling and you are constantly in peril from mentally disturbed people. Whether you play Solo or Multiplayer your only hope is work hard save up your supplies and maintain your night vision goggles battery life.

Violence & Gore
- One of the levels has a man, crucified on a cross with a bag over his head while he is still alive. The objective involves you having to saw off his legs while h screams in agony. After the objective is complete he bleeds out.
- One of the levels takes place in a police department where you must push a man strapped down to an electric chair that is connected to tracks all the way into through the building and into a cage where you power up the electricity. He screams for is life in the process. Once fully powered the man is electrocuted to death. While not to graphic the concept is disturbing especially since you are involved in doing everything.
- In one level there are multiple people who are tied by their feet and hanging upside down right above a grinder. You must find and use keys to lower the people into the grinder while they scream in agony and for there lives. Very bloody and disturbing.
