- Several mutilated female corpses are shown towards the beginning of the film. The breasts of at least one are fully visible.
- A closeup shot of a woman's crotch is shown while she is wearing very tight panties. The outline of her genitals is clearly visible, displaying a cameltoe.
- Breasts, buttocks, and vulva are seen by a nude female character in almost all of her screen time
- At 22:30 a couple is heard having sex off camera. At 22:43, they are seen onscreen thrusting and moaning. Both are completely nude and woman is seen on top of him, closeup breasts. At 23:25 the man is seen completely nude from behind, buttocks visible, while woman is kneeling on the bed, breasts visible. At 24:13 the woman stands is full frontal nudity is seen; after this point she never gets dressed and is shown multiple times full frontal and rear.
- Couples have boisterous and loud sex offscreen.

Violence & Gore

- We see a hospital filled with dead very bloody bodies from a distance, some bodies are missing limbs while a couple are cut in half (from a distance) we see the body of a woman whose chest is open (graphic and bloody) we later see her heart on a table. The walls are covered with blood.
- A villain hits a young man in the back of the head with a pickaxe; his eye pops out and stays on the pointed side of the axe.
- A woman is hit with a shovel in the mouth, the shovel is later pushed cutting her head from the jaw-up (brief but very graphic/bloody)
- Several people are hit with an axe in several scenes, sometimes it's offscreen but most of the time it's graphically on the screen with lots of blood and gore usually resulting in mutilation.
- We see a very bloody/gory dead woman's body, her whole body is bloodied up. Her chest is opened up and her legs are spread apart and her own heart is lying between her legs, words are written in blood on the wall above her head.
- A man's head is impaled with a pickaxe. Blood runs all over his face, the axe is later pulled, blood splatters all over the place as his body drops to the ground.
- A small woman is hit with a pickaxe, the villain thrusts it up through her head from her jaw upwards and then lifts her up into the lights and electrocutes her.
- The villain takes many swings at a woman with the pick axe through a hole in the spring. She misses them all. He then takes the axe in her stomach and goes upward. Offscreen but blood splatter on the wall.
- Many human hearts are seen throughout the movie, sometimes we see several bodies cut open as well.
- A man is pushed towards an axe setting on the floor, the axe impales the man in the eye, and lots of thick blood squirt on the floor.
- A villan with a pickaxe stabs an old man under his chin, it comes out of his mouth. The villain then pulls the axe out and it rips the man's chin/mouth off, it flies towards the screen in 3D and blood is splattered.
- We see a dead burnt body of a woman, her skin is melted all over her face (brief but very disturbing)
- A car crash scene results in two people being mildly injured with one having some blood over face.
- Several very graphic and bloody stabbings with a pickaxe to the torso, limbs and head in several scenes. sometimes people fight with fists/kicks before they go down.
Profanity

- The F-Word is spoken (actually, it's often screamed) close to 40 times.
- The s-word trails behind at about 15.
- Obscene name-calling references oral sex and female anatomy.

- The 2nd scene of the movie is a huge teen drinking party, so everyone is underage and they all have beer and hard liquor.
- Several six packs of beer appear at a party.
- Tom takes unidentified pillsseemingly prescription.
- It's said that several townsmen drank themselves to death.
Frightening & Intense Scenes

- A pickaxe is thrown towards the screen.
- Unsavory characters, many of whom end up getting speared by pickaxe blades, shot, dismembered, and/or eviscerated.
- Human hearts are torn out and put inside chocolate boxes.
- Many jumpy moments throughout.
- Lots of threatening moments including chases and jump scares.

Sex & Nudity
- In one long intense scene of peril, there is female nudity of one character. This starts with couple completing sex in a motel room. We see her breasts and his butt during the sex. Then she finds out he was filming it and yells at him as he puts his clothes on. His buttocks are seen at that time. She then gets up from the bed, and a full frontal angle includes breasts and partially shaved vulva for several seconds as she stands motionless while yelling at him. The man walks out and the woman follows him outside still naked. She follows him through the parking lot for a sequence that is just over a minute, and in the process there are views of her breasts, buttocks and genital region. The man is suddenly killed by another man and the terrified woman runs back into the motel room and is then killed. In a subsequent scene her bloodied body is briefly seen naked an additional time.
- Axel (and therefore moviegoers) views a sex tape that is crime evidence.
- Axel has an affair with one of Sarah's co-workers, getting her pregnant. (She's seen getting dressed after one of their trysts and there is a closeup shot of her crotch in very tight panties - viewers see her cameltoe in closeup as she turns towards the camera).
