- The player character can wear very revealing armour, even nudity is shown on some bosses. Female enemies and NPC's are sometimes hyper sexualized with deep cleavage or exposed breasts.

- Attacking human enemies causes ridiculous amounts of realistic blood to spray and stain the environment and the player. Killing enemies will result in their head, hands, feet, legs, or arms to be brutally sliced or torn off. Gore is often obscured by colorful magic covering most of the screen.

- This game is just as if not more difficult than the first Nioh. So just like the first game it might be intense and/or frustrating for some players.
