Sex & Nudity

  • A female character is shown with scales covering her with the exception of her parts of her stomach and back. Nothing sexual takes place.
  • There's a boss fight where in one phase the boss is covered in scales instead of clothers but no nudity, essentially a lizard with a human head.

Violence & Gore

  • Normal blood splatter, nothing much.
  • You can collect ashes for questlines nothing serious.
  • A sword is depicted with many fleshy and bloody writhing tendrils covering it.
  • While the act is not shown, except for the aftermath, it is extremely implied that a woman has been tortured for many years.
  • There are swords named after body parts, like skullcrusher etc.
  • There's little to no blood but almost no gore. You can turn blood off.
  • Leading into the second phase of one of the boss fights a cutscene plays of a severely mutilated, lunatic man cutting off his forearm. After his forearm is dismembered his arm is turned into a dragon and is used to attack. This scene is part of a skippable cutscene and can be skipped without losing any story elements.
  • The player can stab, impale, slash, smash, burn, and blow up enemies with a variety of in-game weapons, and vice versa. By default, these attacks cause blood to splatter out of players and enemies, however, blood can be reduced or turned off entirely.
  • There is a mechanic known as "hemorrhage buildup", in which certain weapons and abilities can cause enemies to "bleed" after a certain amount of hits. This "bleed" effect is signified by a large blood splatter exploding out of the enemy, and the blood can be seen lingering on surfaces.
  • All gore/blood can be turned off in the settings.

Profanity

  • The words "damn" and "hell" are used multiple times. "Bitch" is used a couple of times.
  • A character says "What a shit-show."
  • A character exclaims out loud, "Marika's tits!"
  • Very infrequent considering how long the game takes to beat and it only curses a few times anyway.
  • Any bad language is in hidden cutscenes most players won't see.

Alcohol, Drugs & Smoking

  • Various potions are seen throughout the game, but there are no drugs or alcohol use.
  • Potions and flasks can be drunk to inflict positive effects on the player.

Frightening & Intense Scenes

  • The game's brilliant art direction is very successful at portraying a spooky, disquieting world and disturbing character design for monsters. All this makes it creepy and non-suitable for pre-teens.
  • There are many disturbing and/or shocking moments. One boss cuts off its arm in a skippable cutscene, and another is depicted eating monsters (no blood or gore is shown).
  • Some of the enemies and creatures feature very disturbing designs. These included a creature with many limbs and a human face inside, a centipede creature, and many other bizzare and disfigured creatures.
  • A monstrous like man begins to viscerally scream in pain as fire consumes him.
  • There are abominations with several limbs that crawl around like a spider and have a paralyzed face like it was frozen as the face was screaming.

Sex & Nudity

  • No nudity, a character once exclaims "t*ts".
  • A hidden character known as Malenia loses most of her clothing in her second phase, with her private areas being covered by strange scabby masses and the rest of her being completely exposed.
  • The player can find an NPC in the Hub named Fia. The player has the option to lay their head into Fia's grasp as she hugs the player. No Nudity but can be unsettling for some.

Violence & Gore

  • The fire giant tears his own leg off during the second stage. Blood is visible and he's heard screaming in pain leaving a visible stump for the rest of the fight. This cutscene is skippable and it doesn't take anything from the story if skipped.
  • A boss who seems like nothing more than a Giant Snake turns its neck around to reveal a Face. Quite disturbing for people with Ophidiophobia. Made even scarier when it's confirmed that the face was once a Man, who literally asked the Serpent to eat him in exchange for Power. If the player returns to the boss room later, a woman is seen eating the Snake whose head is laying there emotionless.
  • A character known as Godrick has multiple hands coming out of his body. This was done by "Grafting" in which he cuts off people's limbs and attaches them to his body. Incredibly disturbing if you think about it.

Frightening & Intense Scenes

  • Enemies include groups of abnormally tall sinister women/witches who creepily dance and cackle nonstop. Their backstory while vague, is still rather spine chilling
  • One section of the game has a castle that is crawling with enemies known as "Fingercreepers" which are creatures made of a bunch of fingers that skitter towards you like a spider. Definitely one of the creepiest things in the game.
  • Other horrifying creatures known as "Abductor Virgins" which are giant machines on wheels that if within close enough range will open up, grab the player with long and twisted arms, and pull them inside to drain the health, giving a serious claustrophobic feeling.
  • Queen Rennala has a legion of students that aren't super difficult to defeat but ominously giggle, sing, smile at the player and crawl around the floor.
  • A creature with dozens of arms which can hit 738 million time a second and scream like creepy old woman This thing is completely optional to fight and also you don't find it a lot in the game.
