- Crew or equipment visible: When Karen Corelli goes to her apartment, thinking she is being followed, you can see the shadow of the cameraman just before she steps up the first step leading to her door.
- Continuity: About 25 minutes in, a man in red trousers walks along the sidewalk. The same shot repeats about 10 minutes later.
- Continuity: At the beginning, in the shooting gallery, the first targets being winched backwards alternate between intact and bullet-ridden.
- Errors in geography: Club 404 is alternatively referred to as being at 184th Street and St. Nicholas Avenue, and being in Harlem. 184th and St. Nicholas is in Washington Heights, north of Harlem.
- Continuity: When Pacino and DeNiro are sitting in Dennehy's office, DeNiro's left collar is protruding outside of his jacket lapel. On the next close up shot, DeNiro's collar is back in place.
- Continuity: When Turk arrives at Spider's club to be set up by the other two detectives, a large sweat stain is in the middle of his sweat shirt. After that scene, it cuts to Pacino on the phone. When the scene cuts back to Spider and Turk a few seconds later, that sweat spot is nearly gone.
- Continuity: During the opening credits, the two main characters fire H&K MP5 submachine guns, but then we see 5.56 rifle casings falling. The MP5 usually fires 9mm pistol rounds. They're rarely used by police, 10mm and .40SW variants exist.
- Audio/visual unsynchronized: After they handcuff Spider to the vault door, Al Pacino puts his gun in his face, and you hear the gun cock. He's holding a Glock, which has an internal hammer, and can only be cocked by pulling back the slide.
- Continuity: When Turk runs out of the lounge in Club 404, alarmed by Rooster shooting Spider, the gold plate on the plaque on the wall next to Turk's head is leaning forward, as it's been stuck into the bottom of the frame, on the outside of the glass. The camera cuts to Rooster holding a gun. When it cuts back to Turk, the plate is in glass of the plaque, a few inches above the bottom of the frame.
- Errors made by characters (possibly deliberate errors by the filmmakers): In the beginning of the movie, when we see the video footage of Robert De Niro's character, he states that he has been a police officer with the NYPD for "nearly 30 years". At the end of the movie, when he is being shown making the video, he states he has been a police officer with the NYPD for "over 30 years".
- Continuity: When our lead stars are in the church (near the beginning after the weapon has been planted) the harsh key light (sun ray from window) comes from stage right. When they cut to the close up it comes from stage left.
