Violence & Gore: 10
Via the light from a small flashlight, we see a man who's chained by the leg in a filthy bathroom. He finds the hacksaw that someone apparently used to cut off his own foot and prepares to do the same to his, but can't go through with it. He then takes a large and heavy bathroom floor tile and repeatedly smashes his ankle with it (with lots of resultant blood). He then grabs his dangling foot and snaps it off so that he can escape. All of the above is in a dimly lit locale, so not everything is seen in full.
We see several flash views of various bloody body parts and a ripped up torso on the floor (from an apparent explosion).
We see a flashback to an ex-con who's standing with various heavy chains attached to various points of his skin, pulling it taut in places and otherwise looking painful and bloody. The TV comes on and Jigsaw informs him that he only has a minute to yank those hooks out of his skin before an explosive is detonated. The man then panics and proceeds to do just that, ripping them out of places all over his body (with very bloody results) as he races against the clock (having seen the explosive results, we know he didn't succeed).
We see a woman suspended off the floor with some sort of attachment connected to her ribs on either side of her body (there's blood there and some on the floor beneath her). The TV comes on and Jigsaw states she can free herself, but only if she reaches her hand down into a jar of acid to remove the key at the bottom that's now starting to dissolve away. She quickly dips her hand in, burning her flesh, but can't get the key. She does so again, eventually grabbing it but not before the acid has turned red from her blood. She yanks her hand out that's now all bloody and tries to undo the lock, but the contraption on her is sprung and it appears to rip open her torso via her ribs (seen from a distance, although we hear and partially see blood pouring down onto the floor below her).
We see a bloody boy in the emergency room who Lynn must work on (he has a bloody head wound, some blood on his side and we see blood on a scalpel Lynn used on him to insert a breathing tube -- the latter is not seen in full detail).
Jeff has a trickle of blood down the side of his head after being injured.
John starts to convulse and then vomits stomach content as well as some blood as Lynn tries to come to his aid.
Jeff tries to reach for a key to free a woman, with his wet face going up against some frozen pipes in a walk-in freezer room. As he then pulls away, a big chunk of skin from the side of his face is torn off (with bloody results and we later see that wound various times).
We see John's blood on Lynn's robe.
Jeff hears a man yelling for help and finds him tied down (by the neck) at the bottom of a circular pit. A tape recording then gives him the rules of this new game: Jeff must burn all of his late son's belongings to retrieve a key that will free the man (who turns out to be the judge who gave the drunk driver a light sentence after accidentally running over Jeff's son). The twist is what will be filling the pit. Large blades start whirling next to it, and we then see large, bloody, and maggot-covered pig carcasses coming out on hooks and being dropped into the blades. The resultant mushy liquid then rushes into the pit, covering the man inside it, up to the point where just his mouth and nose are above the surface of the foul liquid. Jeff then gets the key, gets into the mush, and frees the man just in time.
Lynn is forced to do cranial surgery on John (to relieve growing pressure inside his head), and we see it in full detail, starting with the scalpel cutting the scalp that's then pulled away (with lots of resultant blood). The incision is then washed clean so that we see the actual skull that Lynn then drills down into repeatedly with a power drill (seen in gory close-up, with blood coming out each time, and we hear the sound of the drilling). She then takes a rotary saw and slices through the rest (with blood squirting up onto her face, and we hear the high-pitch sound of the bone cutting), eventually lifting up and holding the square piece of skull. She then makes an incision into the membrane around his brain, all with additional bloody results.
We see a flashback to Amanda cutting into her own leg with a large knife (and see scars from previous such activity).
We see a flashback to John and Amanda setting up a previous "game" scene where he has a lot of real or fake blood on his head and shirt, and then pours real or fake blood onto the floor and then lies down in it.
Jealous of what she thinks is John liking Lynn better than her, Amanda grasps something so tightly that it cuts her hand (we see some blood dripping to the floor).
We see a flashback to Amanda suffocating a person by putting a plastic bag over their head and holding that there as they struggle (we see some blood in that bag during this).
We see a man vomit.
John and another man find a third man in a contraption that a tape recording states will twist his limbs and head around and around unless John is willing to retrieve a key attached to a shotgun that will shoot whoever removes the key. John is reluctant since this is the drunk driver who killed his son, but reconsiders once the twisting starts (we see that the man's hands and feet are impaled in structures that slowly start to rotate, causing him a great deal of pain and the man screams in agony). The twisting continues, including shredding the guy's leg (with bloody results), so John tries to retrieve the key without pulling the attachment. The contraption twists all four limbs and breaks them graphically off. He does so, but the shotgun fires anyway, hitting the second man in the side of the head (with very bloody results). The man being twisted then dies when his head is twisted around, breaking his neck and then breaking his head off completely.
We see a flashback to the guy who smashed his foot off with a floor tile, now limping along and dragging his bloody stump that leaves a trail of blood on the floor.
In a flashback, Amanda's face is very bloody (some even runs from her mouth) after she's been beaten.
One woman shoots another in the back, with a man then shooting the woman shooter in the neck, resulting in both of the wounded being quite bloody (especially the woman shooter). An explosive device around one's neck then detonates and kills her (with bloody/gory results).
We see blood on the bandage around John's head following his brain surgery.
A man and woman's hands are very bloody after she's been shot.
A man uses a rotary saw to slice open another man's throat (with very bloody results).
Keep in mind this is THE MOST VIOLENT SAW FILM in the series.
