  • Sage Stallone turned down the chance of reprising his role of Robert 'Rocky' Balboa Junior, which he played in Rocky V (1990), due to prior obligations with his film company Grindhouse Releasing.

  • According to her gravestone, Adrian Balboa's death was on 11 January 2002.

  • In the scene in the meat factory with Paulie and Rocky, Paulie asks if Rocky is angry because they took down his statue. In actuality, the Rocky Statue was a point of major conflict. The artsy types in Philadelphia, especially those affiliated with the Philadelphia Art Museum argued that the statue was a movie prop and not true art. Their argument prevailed and the statue was moved to the Spectrum, the site still stands today. It was returned to the museum for the filming of future films, but removed shortly thereafter. Eventually, those in charge were moved by pleas of the people and recently returned the statue to the museum. It’s permanent home can be found at the right base of the museum's steps.

  • According to Sylvester Stallone in his DVD Commentary, he did consider bringing back his real-life son Sage Stallone to reprise the role of Robert "Rocky" Junior, but decided against it because he feared people would read too much into the personal conflict between Rocky and Robert, regarding Robert feeling he's "living in his father's shadow".

  • Some of Rocky's dialog with Marie was improvised according to Stallone on the DVD Commentary. Specifically, moments like when he installs the light bulb, when he asks her if she met Step's father on a cruise, and when he stands in the middle of the street and says "I'm smarter than the average bear". On the light bulb scene, he really didn't expect the bulb to be that bright and ad-libbed pretending to be blind along with the humorous dialog. On the "smarter than the average bear" line, Stallone just blurted it out, thus his follow-up line "I don't know what I'm sayin'."

  • Sylvester Stallone trained for six months to get in shape for his role.

  • 'Milo Ventimiglia (I)' claims he got his role in part because of the similarities between his and Sylvester Stallone's mouth. During their first meeting at the time of casting, Stallone noticed Ventimiglia's mouth hooks down in the same way that his does, therefore making Ventimiglia a perfect choice to play his son.

  • Stallone initially wanted Roy Jones Jr. for the role of Mason Dixon. Stallone claims that he left 31 phone messages for Jones but never received a response.

  • Carl Weathers refused to allow his image to be used in the film. A stand-in was used to recreate the archive footage of Rocky fighting Apollo Creed.

  • A scene was filmed in which Rocky sees a woman that looks like Adrian in the building where Robert Jr. works. He follows her for a while before he realizes it is not Adrian. But Stallone decided to cut the scene because he felt that Rocky's loneliness had been established enough.

  • A scene was filmed in which Rocky, after reuniting with Marie, talks with Andy the bartender (Don Sherman) from the previous Rocky films. But the bar scene was completely reshot without Andy. He can be seen very briefly in the background cheering Rocky during the fight.
