Liz is shown in panties and t-shirt, but it's very brief and it's not graphic at all.
- Hellboy grabs a small insect-like creature and squeezes it in his hand until its head explodes in a bloody green mess.- A scene of a room full of bloody mush and unrecognizable body parts. Small insects are seen crawling around in the mess, eating teeth.- Many small insects are seen getting blown to pieces by a very large gun. We see body parts and blood splattering and hitting the camera.- We see the insects pounce on several men. We see one of them getting his face bitten and scratched at, while the other gets wrestled to the ground. In a later scene, the man who was wrestled to the ground has been reduced to a bloody skeleton.- A man with a scarred face attacks a group of guards with a spear-like weapon, we see many guards get stabbed, slashed, and dismembered. Blood is seen squirting out of wounds, and one scene shows the scarred faced man slice a guard's head off. We see the head hit the floor and bounce, leaving a bloody splatter.- Hellboy and Wink (a large tusked troll with a mechanical arm) get in a brawl. We see the two exchange blows until they are bruised and battered. In a later scene, Wink is pulled into a giant grinder. We only hear squishing and crunching, but in a quick scene, we see Wink's legs being sucked into the bloody grinder and see a pool of blood on the floor below the grinder.- A scarred faced man cuts a woman's face with a large knife. We see a bloody cut.- A demon stabs Hellboy in the heart with a spear. We see blood run down his shirt and a small bit squirt out when the man snaps the tip of the spear off inside of Hellboy's body.- A man stabs a king in the chest, and the king turns to stone- Numerous combat sequences, in which participants are killed.

Hellboy smokes a few cigars. Two characters are intoxicated in one scene.
There are a number of fantasy monsters and demons that appear frequently throughout. The title character may even scare very young viewers. There are also a number intense fights, but nothing too graphic.
