Sex & Nudity

- Many female fighters (namely the female ninjas, Sindel and Sheeva) are scantily clad in skimpy outfits that show off their various curves and cleavage.
- Kitana Mileena and Jade wear leotards that are much more cropped than before, leading to a lot of skin being shown.
Violence & Gore

- Per usual with the series, the game is incredibly violent and bloody; kombatants face off in hand-to-hand brawls where bloodshed and gore is rampant by way of heavy blows, multi-touch combos and various special moves.
- A secondary fatality known as the "Animality", carried over from MK 3, shows a winning Kombatant turn into an animal or insect and violently kill his or her fallen foe in a method based on the beast they turn into (e.g. a polar bear mauls an opponent to death, a giant wasp stings a fallen fighter repeatedly).
- A new finishing move, the "Brutality," enables a winning fighter to repeatedly and mercilessly beat on his or her fallen opponent multiple times until he or she explodes in a burst of blood, gore and body parts. The "Brutality" font that appears on screen also has multiple blood splatters for each letter that appears.
- Oddly enough, the Nintendo 64 port can be far more violent and gory than the PlayStation and Saturn editions; animations are faster, combos hit harder, and the "Bloody Kombat" cheat code doubles the amount of blood and gore on screen after a Fatality, with copious amounts of bloodshed, multiple arms, and numerous skulls and rib cages.
- The infamous Fatalities are ever-present, where fallen fighters are killed in a variety of gory, explosive, and sometimes bizarre ways, all of which are still fatal.

- The fatalities are quite frightening but there some funny ones that can be performed, 'Babality' (Opponent changes into a baby) and 'Friendship' (Character does something nice as an act of friendship to his/her opponent).
