- Continuity: The killer's top changes back and forth between orange and black.
- Continuity: Seatbelts and rearview mirrors appear and disappear during the chase scenes.
- Continuity: When Tom puts on his blue shirt, it gets buttoned up too quickly.
- Continuity: When Tom and Jo go to bed first time, Jo has no watch on her wrist, but when they fall in the bed, she's wearing a watch.
- Continuity: Jo isn't wearing a bra at the police ball, but when she undresses she has one on.
- Continuity: Position of bodies and sheets when Tom and Jo are in bed.
- Factual errors: When Tom Hardy arrives at the cabin, he quickly jumps off the boat without securing it to the dock. Later, the boat is still there, which is impossible on a flowing river.
- Continuity: When Tom chases the witness to the Polish Hill murders and tackles him to the ground, Tom's gun switches from the left side of the escapee's face to the right side, and then back to the left.
- Continuity: On the bridge after Jimmy was jumping, Tom's "walking help" was placed on the floor. In the next shot he was using it.
- Errors in geography: Polish Hill is only about 2 miles from downtown. The patrol run for the witness begins on the Allegheny near downtown in broad daylight, and ends in darkness.
- Continuity: When Hardy boards the tug pushing the barge, he racks a shell into the chamber. When he confronts the two thugs holding two men hostage, he asks them to put their guns down. When they don't comply, he racks the pump action again. Since he had already loaded a shell, by manipulating the pump action a second time, he would have extracted the first shell loaded.
- Revealing mistakes: Obvious stuntman for Tom in the "jumping river boat" shot.
- Revealing mistakes: The dead woman lying upside down in the water, blinks after being turned around.
- Factual errors: During the final fight, the underwater sequence shows the water as being crystal-clear. All three of the Pittsburgh rivers (Monongehela, Allegheny and Ohio) are very murky, due to barge traffic and residual pollution.
- Crew or equipment visible: When Hardy is driving the boat the camera is reflected in his sunglasses.
- Continuity: When Tom is taking the boat and Jo to go look for Chicanis, it's still broad daylight when they leave the boundary of their patrol area. It's dark when they get to the place Tom find Chicanis. However, when the kids find a floating body in the park, they arrive at the scene long before the regular police arrives.
- Continuity: When Tom walks on the bridge in the scene just before Jimmy is jumping, Tom can be seen having plaster on his injured leg. In the scene after the jump, Tom is limping to the railing wearing two shoes.
- Continuity: During the opening chase, in close-ups there is a patrol car behind Tom and his father's unmarked car, but in long shots they are the last car in the pursuit.
- Continuity: When both cars crash at the end of the opening chase, the trunk of the killer's car flies open and is badly crumpled when it rolls. Later, we see two officers force the perfectly intact trunk open and a body falls out. The body would have come out in the crash when the trunk opened.
- Continuity: During the chase scene, the killer's Ford loses its front bumper, later on in the chase, the bumper is intact.
- Continuity: In the opening chase scene, the killer's car stops short at the police roadblock and a driver's side hubcap comes off and rolls forward. Later in the chase, both drivers’ side hubcaps are still attached.
- Continuity: Tom damages the front of his unmarked car when driving over the embankment to avoid the big crash near the end of the opening pursuit, but in later shots the car is undamaged.
- Continuity: The killer's Ford car loses the same piece of undercarriage twice when the pursuit is making its way down the hilly street.
- Crew or equipment visible: In the tunnel in the opening chase scene, the killer spins out the police car. Just before the killer rams the police car there is a close-up of the two cars and you can see the camera crew on the right of the tunnel just beyond the barriers.
- Revealing mistakes: When Jo is looking through the newspaper clippings, one headline reads, "City paralized with fear, another body found." The correct spelling, of course, is "paralyzed." Also, close inspection shows that the same text is repeated exactly between articles. For example, the two columns in the clipping entitled "Death toll rises in killing spree" are copied word-for-word in the "City paralized (sic) with fear" article and the "Nurse Paula Puglisi found dead" article.
- Errors in geography: The courthouse scene opens with an establishing exterior shot of the New York State Supreme Court on Center Street in New York City, not Pittsburgh, Pennsylvania where the film is set.
- Continuity: Just before Hardy and the killer falls from the railway bridge into the river the chain on Hardy's handcuffs is long enough for Hardy to strangle his opponent. In all other scenes it has the normal length of only a few inches.
- Continuity: During the final fight scene in the water between Hardy and the killer, there are two shots of a bald Bruce Willis, missing the hair (or wig) that he sports throughout the rest of the movie.
- Crew or equipment visible: When Jo reaches for the case files inside Ton's house boat, you can clearly see a clothes pin (peg) holding back the shirt, probably due to it being another person standing in for her.
- Continuity: When the two cars go off the road and crash at the end of the first car chase, it is broad daylight, yet, when Bruce Willis' character is recovered from under the car, it is completely dark.

- Factual errors: SPOILER: While Tom and Jimmy are struggling in the water at the end, Jimmy pulls out a Taser and fires it, showing the sparks. Firing a Taser underwater would've zapped them both unconscious and they both would have drowned.
- Continuity: SPOILER: On the tug pushing the barge, Hardy shoots the thugs who have the hostages. He leaves the hostages to go after the ship’s captain. As he exits, he is holding his shotgun. After Hardy goes through the door, he is suddenly wielding a handgun.
