- Continuity: When all soldiers are being checked for visual acuity, the headset of one soldier changes from the right side of his head to the left side.
- Continuity: Luc's hair during the showdown with Scott.
- Continuity: Position/size of gag around Veronica's mouth during showdown.
- Errors in geography: The farm near end of movie supposedly in Louisiana, but mountains visible in background.
- Crew or equipment visible: Ramp is clearly visible when Luc drives the car out the window of the motel lobby.
- Continuity: The amount of ammunition on the ammo belt of Scott's M-60 near the end of the motel segment.
- Continuity: When Luc and Scott is fighting in the end of the movie, and when Scott bang his head against the car-window you see he has blood on his head in the first shot, but in the next shot the blood is gone and then appears again in the third shot.
- Crew or equipment visible: During the scenes where Luc is on the prison bus after Andrew kills the driver, the bus goes out of control. During one of the scenes showing the bus weaving back and forth you are able to see the stunt driver steering it.
- Plot holes: Colonel Perry says that the Universal Soldier program is off the books. So why is he doing press conferences about them?
- Continuity: When Sergeant Andrew Scott shoots the Vietnamese boy at the beginning of the film, the boy is wearing a green shirt. But when we see him from the back as Scott shoots him, his shirt is blue.
- Continuity: Luc decoys the soldiers with the wooden "gas station attendant" mascot. But when the truck pulls up, the mascot is still standing where it was when we first saw it. As the UniSols approach, it isn't there any more.
- Continuity: Shoppers in the grocery store stand in the middle of the store with their groceries bagged.
- Continuity: In the canyon scene the weather is partly cloudy, however when the mobile command center falls off the cliff, the sky is clear blue. In the next shot, the sky is clear and taken near sunset, not in the afternoon.
- Continuity: In Deveraux's parents' house, the clock on the mantle reads 8:20 when it is first shown. Several minutes later, the clock still reads 8:20.
- Plot holes: When Luc and Veronica make their escape from the motel, they drive through the office and interrupt the motel owner's mother reading a newspaper. Are we to believe she sat and happily read the paper during the previous sequence when the motel was shot to pieces by 4 heavily armed men?

- Factual errors: SPOILER: When Scott tries to execute Veronica at the farm, there is the sound of the hammer falling on an empty chamber. But as it is an automatic pistol, the slide should have already been locked to the rear, indicating it is empty.
